Enrollment Agreement
BULSAJO TAEKWONDO LLC — ENROLLMENT AGREEMENT
This Enrollment Agreement is entered into between Bulsajo Taekwondo LLC and the undersigned parent/guardian.
TUITION: Monthly tuition is due on the 1st of each month. A $99 annual registration fee is charged at the time of enrollment.
CANCELLATION: There are no long-term contracts. A 30-day written notice is required to cancel enrollment. Tuition remains due during the notice period.
TRIAL: The $75 one-month trial does not auto-convert to a full membership. Students must re-enroll to continue after the trial period.
CONDUCT: A respectful environment is expected at all times. The school reserves the right to dismiss any student for unsafe or disrespectful behavior.
LIABILITY: Martial arts training involves inherent physical risks including but not limited to sprains, fractures, and other injuries. By signing below, the parent/guardian acknowledges these risks and agrees to hold Bulsajo Taekwondo LLC harmless.
MEDIA: By enrolling, you consent to photographs and videos taken during class being used for promotional purposes unless you opt out in writing.
